Servers purchased on vendor recommendation without capacity modeling. Networks configured ad-hoc without threat modeling. Security controls bolted on after incidents — reactive, incomplete, and expensive. Documentation that existed only in the memory of a few key individuals — and vanished when they departed. This pattern was so widespread it had become accepted as normal. We reject that normalization entirely.
Every system we engineer begins with documented requirements reviewed with stakeholders. The architecture goes through formal peer review before any component is procured. Configurations are validated in controlled staging environments before reaching production. The final system is accompanied by formal documentation that any competent professional can use to operate it independently. A system that holds its owner hostage to the original vendor for every routine change is, by our definition, a design failure.
